Effective long-term transitions


All effective long-term transitions involve a period of preparation, followed by the actual move or change and then a process of adjustment and acceptance. Transitioning children with SEND will most certainly be more complex than the support needed for most other children. Children with SEND will need time and planning to support a smooth and positive transition. They may need individual care plans, SEND Support plans and staff may need training on strategies that are already working well such as the use of visuals to support routines and small transitions, intensive interaction techniques or they may need to be trained in the use of medical equipment or emergency medication.
