Plan transitions

At key transitions to a new class or setting,we actively seek and share information about the CYP to support successful transitions and manage change both within the school / setting and beyond.

Key transitions are planned and may involve additional visits.

Transitions throughout the day are planned for and well-managed (e.g. visual timetable, planner) and provide the opportunity for the children and young people to develop the skills to manage change.

We encourage parent carers to consider the steps they can take at home to help prepare their child’s transition to a new setting or class.
